Geriatric medicine

Western Health provides geriatric medicine services to support older adults with memory, mobility and health concerns. Clinics and hospital-based care available.

Geriatric medicine at Western Health supports the health and wellbeing of older adults through specialist assessment, care planning and rehabilitation.

Our multidisciplinary teams provide hospital and community-based services for people experiencing frailty, cognitive decline or complex age-related conditions.

We aim to help older people maintain their independence, dignity and quality of life through coordinated, evidence-based care.

What we do

Geriatricians and allied health teams assess and manage:

  • Memory and cognitive issues
  • Falls and mobility concerns
  • Chronic disease in older adults
  • Functional decline and frailty
  • Medication-related problems
  • Carer stress and safety concerns
  • Planning for future care and support
  • Care is person-centred, collaborative and coordinated across hospital, home and aged care settings.

  • Williamstown Hospital
  • Hazeldean Transition Care
  • Bacchus Marsh and Melton

We offer inpatient consults, specialist outpatient clinics, and support within rehabilitation, subacute and residential aged care programs.

  • Acute Aged Care Services
  • Orthogeriatrics
  • Cardiogeriatrics
  • Geriatric Evaluation and Management (GEM)
  • Secure GEM (Dementia Management Unit)
  • Transition Care – bed-based
  • Subacute and non-acute assessment and pathways

Outpatient services

Specialist clinics include:

  • Continence and Urodynamics
  • Wound clinic
  • Falls and Balance Clinic
  • Cognitive Dementia and Memory Service (CDAMS)
  • Movement Disorders Clinic
  • Geriatric Evaluation and Management (GEM) outpatient services
  • Behavioural assessment and carer support

Community services

The Commonwealth offers a Single Assessment System (SAS), available to people aged 65 and over, and Aboriginal and Torres Strait Islander people aged 50 and over. The assessment determines your eligibility to access Commonwealth government-funded aged care services.

How to access the service

  • Referrals are accepted from:
  • General practitioners (GPs)
  • Hospital clinicians
  • Residential aged care facilities
  • Community health services

Some services require referral forms and clinical information to assist triage.
