Who is Transition Care for?

 Transition Care clients must:

  • be an inpatient of a hospital, having completed their acute stay and any necessary sub-acute care (rehabilitation).
  • be medically stable.
  • agree to admission onto the program.
  • have a completed, current and delegated ACCR (Flexible Care/Transition Care Program must be a recommended service).
  • have potential goals - be able to benefit from a period of stay on the Transition Care Program.​
In the case of a life threatening emergency, call 30020.
