Rehabilitation

We have rehabilitation beds at Footscray Hospital, Sunshine Hospital and Williamstown Hospital.

Rehabilitation is a service for people who need intensive therapy to reach their goals. This can include therapy, education and strategies. We aim to maximise independence and quality of life.

The Rehabilitation team includes:

Referrals to Rehabilitation

Referrals to Rehabilitation are coordinated by the Subacute and Nonacute Assessment and Pathways (SNAP) team .  When in hospital a person's treating team may recommend an assessment by this service to see if they would benefit from a rehabilitation stay.  SNAP can also talk about referrals with local health care professionals and can advise the best way to have an assessment for people who are not in hospital
