We provide acute mental health treatment to older adults in their home or usual place of residence.

What we do

We provide community-based treatment to older adults in Western Melbourne during an acute phase of mental ill-health. 

By providing treatment in the person's home or usual place of residence, we aim to prevent hospitalisation where possible, or minimise the length of an inpatient stay. 

Who can use our service?

Consumers are eligible for ICT if they are an older person who: 

  • Requires acute mental health treatment for which home care is assessed as a safe and acceptable option
  • Lives within the Western Melbourne area 

How we work

We provide intensive services, up to three (3) or more times during the week, including weekends. Active treatment must be required for us to become involved. 

We do not replace the need for community nursing services for functions such as medication administration.

Referrals

We accept referrals for this service via WH Acute Inpatient Unit and APATT.   

If you would like to discuss treatment or care plans, contact ICT on 9057 4150 and ask to speak to the key clinician, Monday to Friday, 9am to 4.30pm.     
 

For out-of-hours support, call Triage on 1300 874 243. If your patient is presenting in an emergency situation, call 000 for an immediate response. 

In the case of a life threatening emergency, call 30020.
