Geriatric Evaluation and Management (GEM) is specialist care for people with chronic or complex conditions associated with:
- ageing
- cognitive problems
- chronic illness
- disability
These conditions require people to stay in hospital for a certain length of time. This gives the team time to complete:
- a review
- treatment
- management
People who need GEM are usually, but not limited to, older people with complex, chronic or multiple health care conditions who have goals for therapy.
The GEM team includes:
Referrals
Referrals to GEM are coordinated by the Subacute and Nonacute Assessment and Pathways (SNAP) team. When in hospital a person's treating team may recommend an assessment by this service to see if they would benefit from a GEM stay. SNAP can also talk about referrals with local health care professionals and can advise the best way to have an assessment for people who are not in hospital.
