What we do

Our Disability Liaison Service provides you with support if you identify as living with a disability. 

Our aims are to:

  • provide you with safer, accessible and more inclusive care during your hospital admissions and outpatient clinic appointments
  • improve your health outcomes
  • shorten the length of your hospital stay
  • reduce re-admissions
  • improve your overall patient experience.

Services and facilities

We are here to help you, your families and carers access services across Western Health and other health and human services as required.

This may include:

  • access to Emergency Department (ED) and Specialist Clinics
  • assistance with Discharge Planning.
  • Community Outreach Support
  • Inpatient Support

We provide the following for you as a member of the community:

  • advocacy
  • advice
  • hospital navigation
  • linking to services
  • NDIS navigation

ED and specialist clinics

Our ED and specialist clinics provide information and advice on:

  • hospital navigation
  • liaising with carer/s, next of kin, and family
  • liaising with Special Disability Accommodation (SDA)
  • linking to services
  • supporting access to specialist clinics appointments

Inpatients services

Our inpatients services include:

  • consultation and advice
  • education for staff on disability support needs
  • help with complex discharge planning
  • linking between disability accommodation, support coordinator and family
  • support during inpatient stay

Discharge services

Our discharge services include:

  • community follow up
  • home visiting post discharge
  • NDIS navigation post discharge.

Referrals

For community and ED and specialist clinics an email and contact number is required.

For inpatients and discharge support an Electronic Medical Record (EMR), email and contact number is required.
