The Community Based Transition Care Program helps people who have gone home from hospital and need extra support while they are settling back into being at home. This support is for a short term period. It can include:

  • Low intensity therapy
  • Organizing support services (for example home help, nursing services, showering assistance)
  • Co-ordinating support services.​
