Following a long hospital stay people sometimes need more time to recover, reach their goals and decide on long term care arrangements in a more home-like environment than hospital.  These people may find the Transition Care Program helpful.

The Dementia Management Unit is located at Sunshine Hospital, Furlong Road, St Albans. It offers clients with Dementia specialised management and support. It also offers more time for people to recover, reach their goals and finalise long term care arrangements in a place specialising in dementia management.
