Diabetes nurse education

Personalised diabetes education and support from trained nurse educators at Western Health.

What we do

Our Diabetes Nurse Educators are specially trained nurses who work with people living with diabetes to help them better understand and manage their condition. 

 

Whether you have just been diagnosed or have been living with diabetes for some time, we're here to support you in taking control of your health and improving your quality of life. 

 

We'll work with you to: 

Understand your diagnosis 

Learn about the different types of diabetes (Type 1, Type 2 and gestational diabetes), what causes it, and how it affects your body. 

Manage your blood sugar levels 

Understand the importance of checking your blood glucose levels regularly, what the numbers mean, and what to do when they are too high or too low. 

Use diabetes devices 

Get hands-on help using devices such as blood glucose meters, insulin pens, continuous glucose monitors (CGMs) and insulin pumps. 

Learn about food, exercise and medication 

Learn how your diet, activity levels and medications all work together to manage your diabetes effectively. We'll work closely with other health professionals such as dietitians and doctors to support your overall care. 

Prevent complications 

Develop a personalised diabetes management plan to reduce the risk of long-term complications such as eye, kidney, nerve and heart problems. 

Support you emotionally 

Living with a chronic condition can be overwhelming. We're here to listen and provide emotional support, reassurance and practical advice. 

Who this service is for

This service is available to anyone living with diabetes, including: 

  • people newly diagnosed with diabetes
  • people who have had diabetes for a while and need a refresher
  • people starting new medications or using devices for the first time
  • families or carers who want to better understand diabetes care. 

Referrals

You can be referred by your GP or another healthcare provider, or you can contact us directly on (03) 9747 7609 to arrange a self-referral.

If you are over the age of 65 (50 for Aboriginal and Torres Strait Islander people), please contact My Aged Care on 1800 200 422 to arrange a referral.
