Western Health is committed to supporting the safety and wellbeing of individuals and families by participating in Victoria's Family Violence Information Sharing Scheme (FVISS) and Child Information Sharing Scheme (CISS).  

These schemes allow authorised professionals and organisations to share relevant information to assess and manage risks associated with family violence and child wellbeing. 

Requesting Information from Western Health

If you are an authorised Information Sharing Entity (ISE) or Risk Assessment Entity (RAE), you can request information from Western Health under the following legislation: 

  • Part 5A of the Family Violence Protection Act 2008
  • Part 6A of the Child Wellbeing and Safety Act 2005

Sharing Information with Western Health

If you have relevant information to share with Western Health under FVISS or CISS, please use the following form:

Western Metro Catchment Considerations

Each hospital in Victoria operates independently with its own governance structure. Patient information is not shared across hospital networks. If you require information from many health services within the Western Metro Catchment, you must submit separate requests to each organisation.
