HOPE Team (Hospital Outreach Post-Suicidal Engagement)

We provide targeted support to people to reduce the risk of suicide re-attempts post-discharge from Emergency Departments, medical wards or inpatient units.

Key points

  • We provide outreach support and clinic-based interventions to people experiencing a suicidal crisis
  • We allocate a Psychosocial Support Worker and Mental Health Clinician to each individual to provide intensive and individualised therapeutic care and psychosocial support
  • Our diverse team of support workers and clinicians can work with you to build your support network and develop individual goals over a 12-week period

What we do

The aim of the HOPE program is to support people for a period of 12 weeks, providing assertive and coordinated care post-discharge, via both outreach and clinic-based therapeutic interventions to reduce the risk of suicide re-attempts and deaths by suicide.

Keeping with the goals of the Victorian suicide prevention framework 2016–25, the Sunshine HOPE team aims to build resilience in individuals who have made a suicide attempt by providing intensive and individualised therapeutic care and psychosocial support.

We offer:

  • 12 weeks of intensive support tailored to your unique needs
  • Development of an individualised safety plan
  • Psychologically focused sessions to increase coping skills
  • Practical support and assistance
  • Connection to community supports
  • Family and carer support

  • Ongoing contact in person or over the phone
