Pathway to Good Health clinic

The Pathway to Good Health Clinic supports children in out-of-home care through medical, developmental and psychosocial assessment and coordinated care.

The Pathway to Good Health clinic is a free service that supports children in out-of-home care (OoHC) who have complex health and developmental needs.

The Pathway to Good Health is a model of coordinated care and health planning for highly vulnerable children. It has been developed and funded by the Department of Families, Fairness and Housing.

Our team at the Pathway to Good Health clinic in Melton includes a paediatrician, speech pathologist, and psychologist.

The role of the Pathway to Good Health team is to provide a comprehensive assessment of the child and develop a health management plan, including any diagnosis, recommended treatment and/or referrals.

The clinic is available to children and young people (0-17yr 11 months old) in Melbourne’s western suburbs and Brimbank Melton who are being actively supported by child protection and not already connected with paediatric care.

Children are referred to the Pathway to Good Health clinic by the children protection.
