What is Transition Care?

The Transition Care Program aims to help older people who have been in hospital by giving them more time to recover and improve their independence and confidence. The Transition Care Program can also be used by people who are waiting for residential care following a stay in hospital.

We aim to reduce premature admission to residential aged care facilities.

Services and facilities

Transition Care packages are available in either a Bed Based setting or in a person's own home (Community Based setting).

The bed based Transition Care Program provides:

  • medical services (including Geriatrician review)
  • 24 hour Nursing care
  • Allied Health (physiotherapy, occupational therapy, speech pathology, dietetics, podiatry)
  • social activities
  • a dedicated Case Manager to assist  clients and their families finalise their long term care and living arrangements
  • more time to recover
  • low intensity therapy
  • a place to stay while deciding long term living or care arrangements
  • a place to stay while waiting for residential care
  • a more home-like environment

 The Dementia Management Unit is located at Sunshine Hospital and offers the Transition Care Program to clients requiring specialised dementia care and support. The community based Transition Care Program can provide:

  • a dedicated case manager to help clients settle back into the community
  • help organising services
  • low intensity therapy at the clients home
  • help co-ordinating services.

The Transition Care Program is a Commonwealth Department of Health and Ageing service and jointly funded by the Commonwealth and Victorian State Department of Health. Transition Care clients in the Transition Care Program also contribute a client daily co-payment which is set at 84% of the single persons aged pension levels. Pharmacy supplies are funded by Transition Care Program clients and these are supplied from a community Pharmacy based in Williamstown.

The Community Based Transition Care Program (home based service) has a daily fee contribution which is set at 17.5% of the aged pension
