The General Practice Integration Unit works to strengthen the link between Western Health and local GPs so patients receive better, more coordinated care.  

 

Our main roles are to: 

  • Improve the way we share information between hospital teams and GPs, especially during inpatient handover and when sending Specialist Clinic updates.
  • Act as a central contact point for GPs who need help accessing hospital services.
  • Collect and respond to GP feedback, and make changes to improve the way we work together.
  • Keep GPs informed about Western Health services through our regular General Practice Newsletter.
  • Give hospital staff advice about working effectively with GPs.
  • Work with community partners to make the transition between hospital care and primary care smoother. 

Updates to clinical correspondence, practices and clinicians

Western Health now uses HealthLink to send clinical documents securely. Since October 2024, we have sent thousands of documents through HealthLink. A full list of these document types is available on the HealthLink Secure Message Delivery for Clinical Correspondence page

 

As of December 2024, we stopped using PulseNet to send clinical documents. Please see the correspondence to GPs and Practice Managers for more details. 

 

While our GPIU team can guide through the process of recovering documents via HealthLink, you will still need to take the steps outlined on this page to make sure you can receive our clinical correspondence.
