Our service

The Discharge Planning Service helps ensure patients receive the right care and support when they leave hospital.

Coordinated by dedicated staff, the service works to create smooth transitions from hospital to home or other care settings, supporting recovery and wellbeing.

 

Who is this service for

Any person seen in either the Emergency Department, Pre-Admission Clinic or admitted to Western Health.

Referrals

Patients can be referred by a medical officer, nursing staff or allied health professional.

In the case of a life threatening emergency, call 30020.
